The Green Beast (AKA Taijutsu Naruto) from the Naruto: Ultimate Ninja games and the Naruto Shippuden filler episode "Ah, the Medicine of Youth" Sadly, Naruto only wore this outfit once in the anime, and Ultimate Ninja 2 and 3. I don't think Naruto with this outfit is in the later Storm games, and Ultimate Ninja 4 and 5. This was a very simple custom (you could probably do this if you have the figures), I had spare figures of Naruto and Rock Lee laying around that I wasn't using, so I just took the body of Rock Lee and put Naruto's head on it.
