Pretty common recipe: Madam Masque body with skirt/skulls from the Toybiz Mystique. The Madam Masque head is similar but slightly less gaunt than the Mystique version, so I sanded and sculpted the head until the mask was seamless with the face, added a sculpted skull at the widow's peak. Reshaped the bust to get rid of the harness and zipper, and fixed her boobs (too much gap in between them) to look a bit more natural. Dremeled out the inside of the skirt so that it fits flush to the figure, and added extra sculpted skulls so that it could sit lower on the hips. Also filled all the cracks and texture of the original skirt piece so that it would look more consistent with the rest of the outfit. Painted everything (even the parts that were already white!) and added shadows and highlights.
